There was some discussion at today’s CO Forum about quick ways of creating simple web forms for collecting information from people which admin offices can then use. Some suggestions:

The final screen dump shows how the replies can be exported as CVS etc. Access to the “Quiz” and replies uses the usual moodle mechanisms.

Stephen S had done a leave form for the Teaching Office web site (http://teaching.eng.cam.ac.uk/node/524). This shows the user data entry bit but he also showed me the page which the Teaching Office can use to look at and export the data and modify the form. It’s pretty flexible.
We also discussed the possibility of using survey type software and a possible locally hostable option that I looked at a bit ago for something else is Lime Survey.
And there are also forms like the helpdesk one on this site though I don’t know how flexible or otherwise these are.

We used Lime Survey for an alumni questionnaire at one point. I’d proposed offering it as a tool for other surveys, but it was overtaken by the Department’s purchase of a licence for SurveyMonkey.
In the version that we used, it was a fairly powerful and flexible system, but it was somewhat complicated to create and configure individual surveys.
The University has an arrangement to use Qualtrics http://cambridge.qualtrics.com, which to my admittedly untrained eye, appears to be comprehensive and fairly easy to learn and use. It produces surveys that are branded with the University style and present themselves correctly formatted for mobile devices.
